Overview

Use this article to confirm the insurance your medspa needs to operate, set up the correct malpractice coverage tier for your medical director, and know who to contact for a policy review.

This applies to any provider using a Moxie professional corporation (PC) or a Moxie medical director. Without proof of the required coverage on file, you can't operate under a Moxie PC or work with a Moxie medical director.

Insurance you need to operate

Your medspa needs two separate types of coverage. The table below shows what each one covers and who it protects:

Coverage type

What it covers

Who it protects

Malpractice (professional liability)

Medical decision-making, supervision, Good Faith Exams (GFEs), chart reviews, prescribing, and adverse medical outcomes

Your medical director, your PC, and providers depending on role

General liability

Slip-and-fall incidents, non-medical bodily injury, and damage to patient or third-party property

Your PC or your management services organization (MSO)

✏️ Note — General liability and property coverage are often bundled together as a Business Owner's Policy (BOP). Property coverage is optional and up to you to add; general liability is required.

How medical director malpractice coverage is tiered

Malpractice coverage for your medical director isn't one flat policy. World Insurance, Moxie's preferred malpractice partner for medical directors, structures coverage in three tiers based on how involved your medical director is with patients. The table below describes each tier and its cost:

Tier

Required when your medical director

Cost with World

Scheduled Physician Endorsement

Handles administrative oversight, protocol review, and compliance documentation only

No fee

Indirect Patient Care Endorsement

Performs asynchronous GFEs, prescribes medications, or reviews charts with patient-specific guidance

$100 flat fee

Direct Patient Care Endorsement

Speaks with a patient directly or is involved hands-on in a procedure

$225 flat fee

Direct patient care is broader than it sounds. All of the following count as direct patient care and require the Direct Patient Care Endorsement:

  • Your medical director speaks directly with a patient after an adverse reaction.

  • Your medical director checks in with a weight loss patient on a recurring schedule.

  • A patient asks to speak with your medical director directly to resolve a concern.

❗ Important — Confirm the Direct Patient Care Endorsement is on your policy even if your medical director rarely interacts with patients directly. If a direct-care conversation happens while your medical director is scheduled at a lower tier, that conversation isn't covered.

Add your medical director and PC to your policy

The table below shows what to add to your policy depending on your setup:

Scenario

Required action

You work with a medical director

Add them to your policy at the endorsement tier that matches their actual involvement with patients, not just "for administrative purposes."

You use a Moxie PC

Add the Moxie PC entity as an Additional Insured.

Your medical director owns the PC

Confirm the PC appears correctly on the policy in addition to the medical director's endorsement.

Treat these as ongoing checks, not one-time setup steps:

  • If your medical director's role changes, for example they start doing patient check-ins they didn't do before, confirm the endorsement on file still matches.

  • If you hire a new provider, confirm the policy gets updated to include them. This doesn't happen automatically just because the hire was approved.

  • If you switch professional corporations (PCs) or medical directors, update the policy to reflect the change. This is a required step on all PC and medical director transition checklists.

Insurance policy review

Moxie requires a review of your current policy, regardless of carrier, before you can operate under a Moxie PC or Moxie medical director. The review confirms your policy includes the medical director endorsement tier that matches their actual involvement with patients, correct PC naming, and correct inclusion of providers, medical directors, and PCs as insureds.

This review is required even if you already have insurance in place.

Using World or your own insurance vendor

World is Moxie's preferred insurance partner for multi-provider practices and practices working with a medical director, because its policies are structured specifically for medspa malpractice risk. You aren't required to use World.

If you already have coverage through another carrier, you can keep it, but you still need to submit your policy for review to confirm it meets the coverage levels required for your PC and medical director. Some non-specialized carriers don't structure medspa malpractice coverage efficiently, which can mean a higher premium for the same level of coverage. If that happens, World can review your coverage gaps and compare policy structures with you.

Renewing and maintaining your policy

Your insurance provider contacts you directly with renewal instructions when your policy is due. A policy doesn't stay active just because it was set up once. Policies get canceled for non-payment more often than expected, and a lapsed policy doesn't announce itself, so confirm your renewal date and payment status directly with your carrier rather than assuming it's handled.

If your policy is with World and is at risk of cancellation, typically for non-payment, World notifies both you and your medical director directly. This gives your medical director visibility to follow up with you before coverage lapses, but it doesn't remove your responsibility to keep the policy paid and current.
